Halofantrine in malaria From aglobal perspective, the needfor new, rapidlyacting antimalarial agents has posed one of the foremost pharmaceuticalchallenges of modern times (1). Overthe past twenty-five years Plasmodium falciparummalaria hasthreatened to escapefrom effectivetherapeutic control, initially in South America andsouth-east Asia (2) and latterly in Africa (3), followingthe emergence of parasites resistant to chloroquine and other rapidly-acting blood schizontocides. In the worst affected areas avirtual collapse in antimalarial therapy has been averted only by resortingto quinine, which earlier had been discarded in the routine management of uncomplicated, acute malaria, both because of its toxicity andthe need for protracted administration. In recent years, however, reported failure rates with quinine used alone have surpassed50 percent in some areas of south-east Asia (4-6). Until recently, the only availablestrategy forstemmingthe risingtide of malaria-related deaths lay in protecting the thera~euticvalueo f quinine, andsome success was claim'ed in suppressi~gthecontinuede mergence of resistant strains by using it routinely in combination with tetracyclines:
